New Member Application
To join, please fill out the member application form (click the PDF icon below). You will need to provide your PTK membership ID number and your PTK induction date. Both are listed on your membership card.
If you do not have your PTK member card, contact:

PTK directly via their website (expect replies between 24-48 hrs.)

When your app is completed, submit it through one of the following:
